A supportive environment where participants work together in a small group (cohort) to learn and implement lifestyle changes such as eating healthier, increasing physical activity, managing stress, and losing a modest amount of weight to prevent or delay development of type 2 diabetes in those at high risk for the disease. |
Program Focus & GoalsResearch funded by the National Institutes of Health (NIH) and the Centers for Disease Control and Prevention (CDC), showed that by eating healthier, engaging in moderate physical activity and losing just 7% of your body weight, a person with pre-diabetes can prevent or delay the onset of type 2 diabetes by up to 71%. That only 14 pounds for a 200-pound adult. Prevent T2 helps participants achieve moderate weight loss by eating well and being active.

Please Note: Referrals, enrollments, classes and coach communications are processed in a HIPAA-compliant environment. Once enrolled, individual attendance and weekly progress results are tracked. Individual results, aggregate data and feedback reports are available and sent to a referring provider on a regularly scheduled basis. |
EligibilityParticipants must:
1). Be 18 years of age or older and 2). Be overweight (Body Mass Index of 25 or more; 22 or more if Asian- see BMI chart HERE) and 3). Not be pregnant and 4). Have no previous diagnosis of Type 1 or Type 2 diabetes plus one of the following:

During the registration process, you will also need to provide your blood test results (within the past year) that are in the prediabetes range or Healthcare Practitioner Recommendation Form (validating a diagnosis of prediabetes or previous clinical diagnosis of gestastional diabetes) or Prediabetes Risk Test score. Then, you will be prompted to make the 1st payment to reserve your space in the program. |
