Distance Learning Cohort 

Distance learning programs are yearlong lifestyle change programs delivered 100% by trained lifestyle coaches via remote classroom or telehealth (i.e., HIPAA compliant Zoom meeting), where the lifestyle coach is present in one location and participants are calling or video-conferencing in from another location.  This is a great option for participants who want group interaction but live in remote areas and cannot attend an in-person program.  Remote classroom/telehealth sessions are one hour and adjustments are made to the schedule as needed for holidays.   
A supportive environment where participants work together in a small group (cohort) to learn and implement lifestyle changes such as eating healthier, increasing physical activity, managing stress, and losing a modest amount of weight to ​prevent or ​delay development of ​type 2 diabetes in those at high risk for the disease.  
Registrants may qualify to participate in this yearlong distance learning cohort for as low as $49 per month based on a minimum of 80% attendance/ participation.  Payments may be charged to the participant's personal credit/debit card or their Health Savings Account (HSA)/Flexible Spending Account (FSA) credit/debit card. People with diabetes tend to develop heart disease at a younger age and diabetes is considered to be a controllable risk factor for cardiovascular disease.

Program Focus & Goals

Research funded by the National Institutes of Health (NIH) and the Centers for Disease Control and Prevention (CDC), showed that by eating healthier, engaging in moderate physical activity and losing just 7% of your body weight, a person with pre-diabetes can prevent or delay the onset of type 2 diabetes by up to 71%.  That only 14 pounds for a 200-pound adult.  Prevent T2 helps participants achieve moderate weight loss by eating well and being active. 

​By the end of the first six months, the goal is for participants to:  
  • Lose at least 5% to 7% of their starting weight
  • Get at least 150 minutes of physical activity each week, at a moderate pace or more
By the end of the second six months, the goal is for participants to:
  • Keep off the weight they have lost
  • Keep working toward their goal weight, if they haven't reach it
  • Lose more weight if they wish
  • Keep getting at least 150 minutes of activity each week.
​Please Note:  
Referrals, enrollments, classes and coach communications are processed in a HIPAA-compliant environment. Once enrolled, individual attendance and weekly progress results are tracked.  Individual results, aggregate data and feedback reports are available and sent to a referring provider on a regularly scheduled basis.

Eligibility

Participants must:
1).  Be 18 years of age or older and
​2).  Be overweight (Body Mass Index of 25 or more; 22 or more if Asian- see BMI chart HERE) and
3).  Not be pregnant and
4).  Have no previous diagnosis of Type 1 or Type 2 diabetes plus one of the following:
  • Have a blood test result in the prediabetes range within the past year:
    • Hemoglobin A1C of 5.7% to 6.4% or
    • Fasting plasma glucose of 100 - 125 mg/dL or
    • Two-hour plasma glucose (after a 75 gm glucose load) of 140 - 199 mg/dL​​
  • ​Have a previous clinical diagnosis of gestational diabetes ​
  • Take the Prediabetes Risk Test and receive a screening result of high risk for type 2 diabetes.

How to Enroll

Select "Register Here" button to enter your contact information, your BMI (based on the BMI chart linked above at requirement #2), and attest that you are not pregnant. 

During the registration process, you will also need to provide your blood test results (within the past year) that are in the prediabetes range or Healthcare Practitioner Recommendation Form (validating a diagnosis of prediabetes or previous clinical diagnosis of gestastional diabetes) or Prediabetes Risk Test score. 

​Then, you will be prompted to make the 1st payment to reserve your space in the program.

The Prevent T2 Lifestyle Change Program is year-long program, based on an evidence-based curriculum, designed for people with prediabetes.  It is also designed for people who are at high risk for type 2 diabetes and want to lower their risk.  ​

Participant's Role

Required:
  • Track & record minutes of activity each day (starting after the Track Your Activity module).  
  • Weigh In and record weight (scale options/recommendations provided)
  • Utilize our HIPAA-compliant web & mobile accessible application for logging (to record required data)
Required During Sessions:
  • Reference participant notebook (online document provided)
  • Attend online, live-streaming class sessions, preferably with camera's on
Highly Encouraged :
  • Track & record food each day (starting after the Track Your Food Module) - utilizing our mobile & web accessible App
  • Track & record more details about activity each day (starting after the Get More Active module) via App
  • Do things asked to try at home
Highly encouraged during sessions:
