For health coaching appointments, a deposit is due at the time of registration to secure and confirm each of your scheduled session(s) with the remaining balance due at the time of your initial appointment and if applicable, up to (4) scheduled sessions reserved. For private class appointments, full payment is due at the time of registration to secure and confirm your place (and materials) for the class.
Requests to reschedule or cancel appointment(s) require notice for a credit (not refund of full payments and deposits) that can be applied to the rescheduled appointment or retained for future service(s) or product(s), Notice is as follows for appointments, classes and online courses:
(24+) hours notice retains 100% of payment as credit to apply to future service or product,
(12+) hours notice retains 50%,
(4+) hours notice retains 25% and
less than 4-hours notice forfeits entire payment.
Additional participants can be included to create a duo, family or small group session(s), as applicable, and additional time may be added from 10-30 minutes per appointment scheduled.
Recreation membership is required for participants to use Liberty Recreation Center or Oak Point Recreation Center for personal training - limited to individual and duo sessions at these facilities.

Based on your selected insurance company, if applicable, you may need to provide a letter of medical necessity with your receipt(s) for HSA reimbursement for health coaching and/or physical activity training. For your convenience, we have provided a downloadable letter of medical necessity here. All payments provide a receipt for your records.
